LEYFI TIL MEÐHÖNDLUNAR PERSÓNULEGRA UPPLÝSINGA / CONSENT TO PROCESSING OFF PERSONAL DATA Ég veiti hér HEI Medical travel samþykki mitt fyrir meðhöndlun persónuupplýsinga um mig og til að deila þeim með viðkomandi samstarfsaðilum svo sem Medical Park sjúkrahúsunum og til að eyða þeim þegar þeirra gerist ekki lengur þörf, samanber lög nr. 90/2018 um persónuvernd og vinnslu persónuupplýsinga og reglugerð Evrópuþingsins og ráðsins (ESB) 2016/679. I hereby give HEI Medical travel specific, unambiguous consent to process my personal data and to share it with relevant co-operators as suppliers of healthcare service such as the Medical Park hospitals and to delete it when it is not needed any more at HEI, as defined in Icelandic laws no. 90/2018 on personal protection and processing of personal data and Regulation (EU) 2016/679 of the European Parliament and Council.
